Last updated March 1, 2026. Please read these terms before using the Woody Folsom website, requesting a visit, or joining a membership plan.

1. Agreement

These Terms & Services are a contract between you and Woody Folsom for the use of woodyfolsomvidalia.com and the informational services described on it. By browsing, sending a form, requesting a membership, or reading the health journal, you agree to these terms. If you do not agree, please leave the site and do not send personal information through it.

These terms cover the public website. A separate clinical consent may be used before an examination, a procedure, or a treatment plan. If a clinical consent and these website terms ever speak differently about medical care, the clinical consent governs the care, and these terms govern the website.

2. Who may use the site

The site is intended for adults arranging their own care or acting for a child or adult for whom they have authority. If you use the site for someone else, you confirm that you are allowed to share their information and to request a visit on their behalf. You must not use the site if a court or a clinician has told you not to contact us, or if you intend to misuse clinical information.

We may refuse, pause, or close website access when we believe these terms have been broken, when a request poses a safety concern, or when we need to protect other patients and staff. Closing website access does not, by itself, end an existing clinical relationship. That relationship is handled by the care team.

3. The website is not emergency care

Woody Folsom's website is an educational and scheduling aid. It is not an emergency service, not a crisis line, and not a substitute for in-person assessment when symptoms are severe. Do not use a form to report chest pain, trouble breathing, a seizure, a serious injury, uncontrolled bleeding, a sudden neurological change, or thoughts of harming yourself or someone else. Seek local emergency services for those situations.

Messages confirmed on a form are reviewed in ordinary care hours. A delay can happen on weekends, holidays, and whenever volume is high. If your condition changes while you are waiting, seek urgent or emergency care rather than sending another note.

4. No clinical relationship from browsing

Reading articles, looking at clinician profiles, or comparing membership plans does not create a clinician-patient relationship. A relationship begins only when a Woody Folsom clinician accepts you for care, typically after identity is confirmed and a visit or a formal telehealth encounter is agreed. Until then, journal articles are general education. They do not diagnose you, choose a dose, or tell you to stop a medicine.

Clinician biographies describe interests and roles. They are not a promise that a named clinician is available on a particular day, nor a guarantee of a particular outcome. Medicine involves uncertainty. We commit to careful reasoning and clear explanations, not to a specific result.

5. Appointment requests

The appointment form asks for your name, a phone number, a record number if you have one, a preferred date and time, a reason, and a department. You agree that the information is accurate to the best of your knowledge. A request is not a confirmed visit. Confirmation happens when we reply with a time that you accept.

Please arrive prepared with a medicine list and your questions. If you need to cancel, tell us as early as you can so another patient can use the time. Repeated missed visits may lead us to ask for a different scheduling approach. We may ask you to see the appropriate specialty rather than the clinician you first named if that is safer.

Do not include highly sensitive details you would not want stored in ordinary email. Share the minimum needed to match you with the right team. Clinical detail belongs in the visit.

6. Membership plans

Pricing pages describe membership bundles such as family care, wellness, continuing care, recovery, women's health, and active life. Amounts are shown per month in the currency marked on the page and can change after notice on this website. A plan lists the kinds of visits and coordination it includes. It does not include every laboratory test, image, hospital stay, or medicine, unless a written estimate says so.

Membership is not insurance. It does not pay other facilities, and it is not a promise that a treatment is medically appropriate. Clinicians may recommend care that sits outside a plan. You may decline that care. If you join a plan, you agree to pay the published amount for the period you selected and to read the description before you rely on it.

You may ask to change or end a plan by emailing hello@woodyfolsomvidalia.com or by telling the desk at a visit. We will confirm the date the change takes effect. Amounts already earned for a period underway are handled as explained in that confirmation.

7. Health journal and other content

Articles are written and reviewed for general education. They discuss anxiety, immunizations, heart habits, pain, diabetes, movement therapy, stigma, allergies, skin, and similar topics. Knowledge changes. We date significant updates, but a page can lag behind a new study or a change in clinical practice. Always confirm advice with the clinician who knows your history.

You may not copy our articles, design, or logo for commercial use without written permission. You may share links. Short quotations for personal or classroom discussion are welcome if you credit Woody Folsom and link to the original page. You may not present our content as your own clinical service.

8. Acceptable use

You agree not to misuse the site. Misuse includes attempting to break security, probing forms for weaknesses, uploading harmful code, harvesting email addresses, impersonating a clinician or a patient, sending spam through the subscribe tools, or using the site to harass staff. You also agree not to submit another person's information without authority.

We do not allow content, in comments or messages, that is unlawful, hateful, or sexually exploitative, or that encourages dangerous self-treatment. We may remove comments and block further notes. Patient stories shown on the site are illustrative or used with permission. Do not post another patient's story in a comment.

9. Intellectual property

The Woody Folsom name, shield logo, page design, photographs we commissioned, and original text are owned by Woody Folsom or used with permission. You receive a limited, revocable license to view the site for personal health information. You do not receive ownership of any clinical method, trademark, or design. Feedback you send about the website may be used to improve it without obligation to pay you, provided we do not publish your medical story without permission.

10. Privacy

Our Privacy Policy explains what the website collects and how forms behave. It is part of these terms. Because this is a static website, form checks happen in your browser. Do not assume a form has delivered a medical record to a chart unless you have received a personal reply. For privacy requests, email hello@woodyfolsomvidalia.com.

11. Third parties and links

Share buttons open services operated by other companies so you can tell someone about a page. Those companies have their own terms and privacy practices. A link does not mean we control or endorse the destination. Journal content may mention general categories of care without endorsing a product brand.

12. Disclaimers

The website is provided as available. We work to keep it accurate and reachable, but we do not warrant that it will be uninterrupted, error free, or fit for a purpose you did not discuss with a clinician. To the extent the law allows, we disclaim implied warranties that cannot be excluded only where the law forces them to remain. Nothing in these terms limits liability that the law does not allow us to limit, including liability for harm caused by negligence where such a limit would be invalid.

You are responsible for how you use general education. Delaying emergency care because you were reading an article is not a use we authorize.

13. Limitation of liability

To the extent permitted by applicable law, Woody Folsom and its clinicians and staff are not liable for indirect or consequential losses arising only from website use, such as lost time from a scheduling misunderstanding, when you had a reasonable way to seek urgent care. Direct clinical care is governed by the standards and consents that apply to that care, not by this website limitation. Some laws do not allow certain exclusions. In those cases, our liability is limited to the smallest extent the law allows.

14. Indemnity

If you misuse the site, submit information you had no right to share, or use the content to offer medical advice to others as if you were Woody Folsom, you agree to cover reasonable losses and costs that result, including legal costs, except to the extent the loss was caused by our own negligence or misconduct.

15. Changes

We may update these terms when services, plans, or the law change. The date at the top will change. Continued use after an update means you accept the new terms for future use. If a change materially reduces your rights, we will also note it on the contact or home page for a reasonable period. Clinical consents already signed are not rewritten by a website update.

16. Disputes and interpretation

These terms are interpreted under the laws that apply where you live and where Woody Folsom operates, without us naming a single venue in this public page. We would rather resolve a concern directly. Email hello@woodyfolsomvidalia.com with a clear description and give us a reasonable time to respond before starting a formal proceeding, unless the law gives you a right that cannot be delayed. If a section of these terms is found unenforceable, the rest remains in effect.
